The Chicago Department of Public Health (CDPH) has announced the opening of Phase 1A and 1B COVID-19 Vaccinations to Healthcare Workers, First Responders, Frontline Essential Workers and Residents Age 65 and over. Sinai Chicago is now offering COVID-19 vaccinations to Phase 1A and 1B individuals who reside in the City of Chicago. Healthcare Workers, First Responders, Frontline Essential Workers and Residents Age 65 and over can proceed to complete this registration form.
